HEALTH PLANS

MEDICAL INSURANCE

• Deductibles:
   Individual: $3,500.00

• Prescription Drug Coverage: Prime Therapeutics
• Premiums: (Per pay period)
   Employee only: $41.65
   Employee + Child(ren): $157.10
   Employee + Spouse: $168.74
   Family: $259.65

Note: Medical coverage does apply a spousal exclusion if your spouse is eligible for benefits through his or her own employer.

HEALTH SAVINGS ACCOUNT (H.S.A.)
• Enrollment in the Blue Cross Blue Shield coverage is required to participate.
• Plan allows employees to set aside pre-tax dollars into a medical reimbursement account to pay medical out of pocket expenses.
• IRS maximums and eligibility requirements apply
• Employer Contributions available semi-annually.

DENTAL INSURANCE
• Carrier: Delta Dental of Illinois
• Benefits are per calendar year (Jan 1 – Dec 31)

RETIREMENT PLAN

401K
• Administrator: TransAmerica Retirement Solutions (TRS)
• Available for all full-time and part-time employees age 21 and over.
• Employees may contribute as much as 75% of their annual salary up to the yearly maximum amount established by the IRS.
• Eligible on the first day of employment. No waiting periods.
• Auto enrollment is available after 60 days of employment if you do not make an election by that time.
• Employer contributions available upon approval from the board of directors.

TUITION REIMBURSEMENT
• Full-time employees are eligible for tuition reimbursement per approval from their General Manager.
• 50% tuition reimbursement is granted per a grade of “C” or better at an accredited educational institution.

PAID TIME OFF

 VACATION
Full-time employees are eligible for vacation based on the following schedule:

  • Year of hire: 1 week of vacation if hired before October 19
  • 2nd Calendar year: 2 weeks of vacation
  • Calendar year after 5th Anniversary: 3 weeks of vacation
  • Calendar year after 15th Anniversary: 4 weeks of vacationNote: One week is defined as the weekly hours an employee is regularly scheduled to work.

PERSONAL DAYS
• Full-time employees are eligible for 2 personal days in the calendar year following their hire date (Jan 1).

SICK LEAVE
• Full-time employees are eligible for 7 days of sick leave after 90 days of employment.
• Sick leave is prorated from your hire date.

LIFE & DISABILITY BENEFITS

BASIC LIFE INSURANCE

• Coverage: 1.5X your annual salary
• Premiums: Provided by the company at no charge to the employee.

ADDITIONAL LIFE INSURANCE
• Carrier: Standard Insurance Company
• Guaranteed Issue Amounts
   Employee: $170,000.00
   Spouse: $25,000.00
   Dependent Children: $10,000.00
• Premiums: Vary based on the applicants age and the amount of coverage elected.

ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D)
• Carrier: Standard Insurance Company
• Coverage: 1.5X your annual salary
• Premiums: Provided by the company at no charge to the employee

SHORT TERM DISABILITY (STD)
• Provided by: The Standard
• Eligibility begins on the 1st of the month following 60 days of full time employment.
• Coverage: 70% of your Salary
• Waiting Period: 7 calendar days
• Maximums: As Certified by your physician but not to exceed 179 days.
• Premiums: Provided by the company at no charge to the employee
